Indications: Bij directe type (IgE gemedieerde) en ernstige uitgestelde* (niet IgE gemedieerde) penicilline allergie

	Medication: vancomycine iv loading dose 15mg/kg, 30mg/kg continuous per 24 hours	Remarks: Vancomycine continue alleen via centrale veneuze toegang. Indien geen centraal veneuze toegang: vancomycine intermitterend doseren. Zie: vancomycine dosering (MeanderMC)

General comments

Behandelduur 12 weken

Bij gebruik van vancomycine langer dan 14 dagen kan ototoxiciteit optreden. Maak bij klachten zoals gehoorverlies of tinnitus laagdrempelig een audiogram.
