Alternatief: vancomycine iv 15 mg/kg 2 dd Alternatief: flucloxacilline iv 2 g 6 dd Zie: vancomycine dosering (MeanderMC) Antibiotica pas starten na afname van 3 bloedkweek setjes. Neem contact op met arts-microbioloog om incubatieduur van bloedkweken te verlengen. |
