General comments
start bij voorkeur iv, bij goede respons oraal uitbehandelen.
Bij obesitas (=BMI>30kg/m2) en dosering ≥ 10mg/kg (iv): om toxiciteit (m.n. nefro- en neurotoxiciteit) bij obese patiënten te voorkomen, doseer op basis van het “adjusted body weight” (zie: https://www.mdcalc.com/ideal-body-weight-adjusted-body-weight). Monitor na start urineproductie en dagelijks de nierfunctie. Bepaal tevens een aciclovir dalspiegel 2-3 dagen na start. Bij twijfel over dosering, overleg laagdrempelig met de dienstdoende ziekenhuisapotheker.
