General comments

  • Orale uitbehandeling na minimaal 1 week intraveneuze therapie en op geleide van gevoeligheid. Mogelijkheden in volgorde van voorkeur: amoxicilline po 1 gram 3 dd, levofloxacine po 500 mg 2 dd, doxycycline po 100 mg 2 dd; linezolid po 600 mg 2 dd, maximaal 4 weken. Indien geen van de andere orale middelen een optie is, dan is ter behandeling te overwegen: vancomycine iv 8 weken + linezolid po 4 weken.
  • De combinatie doxycycline + rifampicine is ongewenst vanwege interacties die kunnen leiden tot te lage concentraties van doxycycline. Kies bij patiënten die ook rifampicine moeten gebruiken voor minocycline po 100 mg 2 dd + rifampicine.
  • Bewijs voor toegevoegd effect van rifampicine ontbreekt.
  • Totale behandelduur 12 weken.
