Indications: Bij directe type (IgE gemedieerde) en ernstige uitgestelde* (niet IgE gemedieerde) penicilline allergie

Alternatieve antibiotica bij resistentie

Flucloxacilline resistentie: vancomycine iv 15mg/kg 2dd. Vervolgdoseringen altijd op geleide van spiegels. Zie: vancomycine dosering (MeanderMC)
Levofloxacine resistentie: afhankelijk van gevoeligheid, clindamycine po 600 mg 3dd, cotrimoxazol (trimethoprim + sulfamethoxazol) po 960 mg 2 dd, doxycyline po 100 mg 2dd.
Rifampicine resistentie of overgevoeligheid: bij orale uitbehandeling is clindamycine po 3 dd 600 mg monotherapie de eerste keuze indien gevoelig. Alternatieven hiervoor zijn levofloxacine 2 dd 500 mg + doxycycline 2 dd 100 mg of cotrimoxazol 2 dd 960 mg monotherapie.
De combinatie doxycycline + rifampicine is ongewenst vanwege interacties die kunnen leiden tot te lage concentraties van doxycycline. Kies in dat geval voor minocycline po 100 mg 2 dd + rifampicine.
