Remarks:

Gentamicine in principe eenmalig in afwachting van kweken tenzij pre-existente eGFR <30 ml/min. Bij een eenmalige gift geen TDM. Maximaal 3 giften.

General comments

Stroomschema verdenking HAP

  • Bij empirische antibiotische keuze rekening houden met kolonisatie/eerdere kweken.
  • Orale uitbehandeling op basis van recente sputumkweek. Indien geen recente sputumkweek aanwezig uitbehandelen met amoxicilline + clavulaanzuur po 875/125mg 3dd
  • *AmpC-producerende bacteriën: Klebsiella aerogenes, Enterobacter spp., Serratia spp., Providencia spp., Citrobacter freundii groep, Morganella morganii of Hafnia alvei. Gedurende een behandeling met cefalosporines (o.a. cefuroxim, ceftriaxon en ceftazidim), piperacilline/tazobactam of amoxicilline/clavulaanzuur kunnen deze bacterien snel resistent worden tegen het gegeven antibioticum.

Gentamicine

  • I.v.m. de toenemende resistentieproblematiek is het toevoegen van gentamicine aan cefuroxim van belang voor het verbreden van het spectrum
  • Gentamicine in principe eenmalig in afwachting van kweekuitslagen. Maximaal 3 dagen.
  • Doseren indien eGFR >60ml/min:
    • Gewicht tot 100 kg: gebruikelijke dosering (5 mg/kg TBW)
    • 100 - 120 kg: 480 mg 1x per dag
    • 120 - 140 kg: 560 mg 1x per dag
    • 140 - 160 kg: 600 mg 1x per dag
    • 160 - 180 kg: 680 mg 1x per dag
    • 180 - 200 kg: 760 mg 1x per dag
    • 200 - 220 kg: 800 mg 1x per dag
  • Doseren indien eGFR 30-60 ml/min:
    • Gewicht tot 100 kg: gebruikelijke dosering 5 mg/kg TBW
    • Gewicht >100 kg: 2 mg/kg TBW
  • Patiënten met een pre-existente eGFR <30 ml/min hebben een contra-indicatie voor behandeling met gentamicine
